This document can be used to obtain alternative contact information for each resident. Alternative contact information could be cell phone numbers, email addresses, etc. This allows you to email or text information to each resident and to provide documents in electronic format versus paper except as required by regulation.
Live-in Aide Addendums are strongly recommended by HUD. Any new addendum must be approved by HUD before you begin using it. If you wish to implement a Live-in Aide Addendum, this version may assist you in developing your own version. This IS NOT a pre-approved addendum. You must obtain approval before implementing.
This lease addendum was designed for use when a RAD resident is relocating (for rehab) to another unit under the same contract and when the resident is expected to return to the same unit in a short period of time.
Owner/agents should advise applicants (not residents) when the tenant selection plan undergoes significant change. Owner/agents must advise applicants if the owner/agent or HUD adds preferences or when HUD changes eligibility requirements. This is a sample notice explaining any preference changes. This notice also inquires as to the applicant’s interest in remaining on the waiting list.

For Component 1 PH to PBRA RAD, certification information must be converted from HUD Form 50058 to HUD Form 50059. Because the two forms collect slightly different information, it is good to use one questionnaire to capture required data.
For PBRA RAD with LIHTC, certification information must be converted from one program to another and an Initial TIC must be created for LIHTC use. It is good to use one questionnaire to capture required data.
This sample worksheet allows owner/agents of Component 1 PBRA RAD properties to bill for rehab assistance. Please note that proration must extend six decimal points.
Use this sample form if you want to require that residents register their vehicles.
Anytime a member cannot provide an SSN or adequate documentation to verify the SSN, owner/agents should advise the member of the requirements. This form can be used to communicate HUD's rule. This is very important when a minor under the age of six is added to a household because the family has 90 days to provide the SSN and adequate documentation to verify the SSN. Failure to do so will result in termination of tenancy.
Use this form to document that all required forms have been provided when converting to PBRA RAD.
Owner/agents are required to ask all adults if they are a student enrolled in an institute of higher education (full time or part time). If the answer is yes, student eligibility must be determined. The questionnaire is a tool created by the owner/agent. There is no HUD requirement to have a questionnaire. This form was completely revised based on recent changes and customer comments.
If your property does not have a senior designation and when you would like to give preference to seniors, you must request permission from HUD. This sample letter can be used to help create your own request for an elderly designation.
