Notice Term Assistance Ineligible Student

The questionnaire is a tool created by the owner/agent. There is no HUD requirement to have a questionnaire. Use this form to verify if income from a “work study” or Job Training program should be excluded from the family’s income calculation. If the verifier answers “yes” to any of the clarification questions, then the income should be excluded based on guidance provided in HH 4350.3 R1, Exhibit 5-1.
Verification Student Certification Parent Income

Section 8 Only. HUD does not provide a “HUD approved” student verification form. It is not necessary to use this form if the student is otherwise eligible. This form is used to when the student is only eligible because the parent/guardian income is no higher than low income limit where he/she lives. There is no requirement to verify the parent’s/guardian’s income other than self-certification unless the owner/agent suspects misrepresentation.
