Self Certification Anticipated Child

This form can be used by a resident to self-certify the upcoming addition of a child as a result of pregnancy, adoption or fostering. Per HH 4350.3 family self-certifications must be notarized or witnessed.

Self Certification Assets

While HOTMA is effective 1/1/2024, Streamlined verification of Assets was introduced to HUDs Multifamily Housing Programs in 2019. Owner/agents may, but are not required to, adopt Streamlined Verification of Assets. This means that the cash value of assets and income from assets is verified in Year 1. Under HOTMA, when the total cash value of all family assets totals $50000 or less (may be adjusted for inflation), owner/agents may accept self-certification of the cash value of assets and income from assets in Year 2 and Year 3. Use this form to obtain information about assets from residents at Annual Recertification (and Interim Recertification as necessary). Note from RBD: We do not expect additional revisions to this rule.

Self Certification Current Assistance

Use this notice to advise applicants that they must forfeit any current HUD assistance before they will begin receiving HUD assistance for a unit on your property. Based on customer comment, this Notice has been edited and is now a form to be complete by applicants that can be included as part of the Application Package or part of the application process.

Self Certification Dispute EIV Social Security Income Accuracy

The form is used to document residents' response to inquiries about Social Security income listed on various EIV income reports. This can also be used as self certification of income when investigating income discrepancies.

Self Certification Divestiture of Assets

This document can be used by resident to certify whether or not any resident has disposed on any assets for less than fair market value. Use this document when the property is not using Streamlined Verification of Assets.

Self Certification EIV Employment New Hire

The form is used to document residents' response when there is new employment, the employer will not respond to verification requests and the resident does not have proper documentation (not enough pay stubs). This advises the resident that the owner/agent will not delay IR.

Self Certification EIV Income Accuracy

When reviewing EIV Income Reports, owner/agents may want residents to verify whether or not the income reported in EIV is correct. This form can be used when a resident agrees or when a resident disagrees with the information provided in EIV for employment, unemployment and Social Security benefits.

Self Certification EIV SSN

Because SSA information is not perfect, sometimes EIV returns an error for an SSN and the error is invalid. Use this form to document discussions with residents regarding EIV SSN errors.

Self Certification Fixed Income Family

Self Certification Income

Streamlined Certification for Fixed Income Families has been available for HUD’s MFH programs since 2019. This is not new with the implementation of HOTMA. This self-certification allows residents to disclose any and all income for family members. If an owner/agent has adopted Streamlined Certification for Fixed Income Families, owner/agents have the option of accepting self-certification for any income that is not fixed.

Self Certification Income Irregular Income

The RBD EIV Policy Template allows owner/agents to use historical information in EIV to estimate income when a resident has irregular income. Since EIV cannot be used to verify income, the resident must self-certify. This method is used when no other acceptable method is available and attempts to obtain 3rd party verification must be documented in the tenant file.

Self Certification Income New Employment

The form is used to document residents' response when there is new employment, the employer will not respond to verification requests and the resident does not have proper documentation (not enough pay stubs). This advises the resident that the owner/agent will not delay IR.

Self Certification Income No 3rd Party Emp

Self Certification Joint Custody

This form is used when two families share custody of one child AND BOTH FAMILIES RECEIVE HUD HOUSING ASSISTANCE. This form helps property managers gather the information needed to correctly calculate the assistance payment including the percentage of time the child lives in your unit, unearned income and appropriate deduction information.

Self Certification Need for Hardship Exemption

Use this form to document a residents request for your to waive the $25 minimum rent. Section 8 only.

Self Certification No Child Support

Use for residents who must certify that they do not receive child support. This form can be used as an affidavit that the resident does not receive awarded child support and they are pursuing it.

This self certification can be used when an applicant has no formal rental history.

Self Certification No SSN

Anytime a member cannot provide an SSN or adequate documentation to verify the SSN, owner/agents should advise the member of the requirements. This form can be used to communicate HUD's rule. This is very important when a minor under the age of six is added to a household because the family has 90 days to provide the SSN and adequate documentation to verify the SSN. Failure to do so will result in termination of tenancy.

Self Certification No Third Party - Employment/Income Change

Use this self-certification when a resident’s employment changes or when income changes (such as a new bonus). Generally, this is used when the resident receives some sort of bonus or one-time payment.

Self Certification of Accuracy of Employment Information In EIV

The form is used to document residents' response to inquiries about employment listed on various EIV income reports or the New Hires report. This can also be used as self certification of income when investigating income discrepancies.

Self Certification of EIV Employment Discrepancy

Use this self-certification when a resident will self-certify that the employment income information displayed in the EIV Income Report is correct and will be used to correct a prior cert or insert a retroactive certification. Generally, employment income information in EIV may only be used as a verification source when an owner/agent is investigating an income discrepancy. See HSG Notice 13-06 Attachment 5.

Self Certification Payee - Use of Funds

Owner/agents may use this form when a resident receives benefits but those benefits are paid to a Representative Payee. The income must be associated with the recipient (not the Representative Payee) and the asset is essentially owned by the recipient (not the Representative Payee). When a minor is part of a joint custody arrangement, the income and the asset are associated with the family taking advantage of the $480 dependent deduction.

Self Certification Real Property Exemption

Under HOTMA, Section 8 residents must disclose if they own real property suitable for occupancy and if they have the right to sell the real property. If so, the family is not eligible to continue to receive Section 8 housing assistance. There are some exceptions. If a resident or applicant owns real property but feels they are exempt under the rules, this form can be used to obtain self-certification and other information about the exemption. Eligibility review based on the Section 8 Asset Restrictions (Asset Cap and ownership of Real Property) are mandatory at application. Owner/agents must decide whether or not to enforce these restrictions after move-in.

Self Certification Receipt of Required Documents at AR

Use this form to document that all required forms have been provided at AR. As of 9/1/2024, HUD has not provided the new 9887 Package or the new HUD FACT Sheet – How Your Rent is Determined. Once those forms are released, owner/agents will update the FACT Sheet but still distribute each year at AR. Once the new 9887 is released, owner/agents will be required to provide this document 1) when an applicant is at the top of the waiting list (eligibility interview), 2) when a new adult is added, 3) when a minor turns 18 and 4) if subsidy is terminated then reinstated.
